Greetings and welcome to Integrated Sleep Center. We are located 5421 Broadway, San Antonio. Parking is located in the rear of the building. Please arrive at your scheduled time. If you must cancel for any reason, please contact us immediately. Please complete the enclosed forms prior to your visit. This provides valuable information to the physicians, regarding your sleep history. Your “Sleep Dairy” needs to be completed on a daily basis for 2 weeks, if possible.
Bring the following with you to your consultation.

1. Sleep Questionnaire, completed
2. Sleep Diary completed on a daily basis for two weeks
3. Insurance card
4. Drivers License or identification

After your appointment with the physician, you may be scheduled to return at a later date for an overnight sleep study in our sleep center. We will give you an appointment while you are at our office. If you have any questions, please contact me at (210) 824-9919.
